Requests from civilian providers must include the doctor’s license, number, telephone, and fax number. Orders can be hand carried or faxed to 208-828-3793.
Referrals for Radiology Services We Don’t Provide
See your primary care manager to request a referral for Radiology services we don’t provide. If your PCM refers you to a specialist, that specialist may request authorization from TRICARE for tests related to the condition for which you were referred
